The placeholder set-up does seem limited.
Did you remove the WP placeholders? Can you make sure they are still enabled through the Ezoic WP plugin? https://support.ezoic.com/kb/article/setting-up-ad-tester-placeholders-with-the-ezoic-wordpress-plugin
You can also add more placeholders with the Ezoic Chrome extension. https://support.ezoic.com/kb/article/using-the-ezoic-chrome-extension
